Case Statement
Rosa Perez is a 12-month-old Hispanic female presenting with her mother for a routine 12-month well-visit. Her overall health is reported as “pretty good,” with no recent acute or chronic infections. Rosa has no known allergies and is not taking any prescription medications. All immunizations are up to date.
Rosa is developmentally appropriate for her age, having transitioned from breastfeeding to self-feeding with finger foods and drinking from a cup. She consumes milk daily, eats three meals per day, and attends full-time daycare during the week. Her mother reports multiple wet diapers daily and uninterrupted sleep, including daytime naps. There have been no changes in Rosa’s appetite.
The primary concern is the new onset of dry, red, and itchy skin patches, first noticed approximately three weeks ago. The rash began as small lesions behind her knees and has since worsened, spreading to the creases of her elbows. Her mother describes the rash as itchy but not painful, with no associated symptoms such as fever or systemic changes. The condition has not responded to the application of lotion, and there have been no recent changes in skincare products, detergents, or soaps. There is no family history of similar rashes.
Rosa’s overall presentation is consistent with a well-child visit, with the exception of the described dermatologic issue.
