Nina Adams is a 45-year-old female who presents with complaints of left eye decreased vision and right arm weakness. She related a short bout of diplopia of around 8 weeks ago, which spontaneously resolved after 2 hours. PE is positive for left eye 20/200 vision, optic disk edema and pallor, right arm weakness (4/5) and decreased tactile sensation. PMH is positive for well controlled HTN. ROS is positive for profound fatigue, chronic cough, and occupational exposure to repetitive motion.

Nina Adams, a 45-year-old female, arrives at the emergency room with complaints of fatigue and progressive weakness in her right arm and hand. Her symptoms began spontaneously two months ago and have gradually worsened, significantly affecting her ability to perform repetitive movements and heavy lifting at work. Two days before her visit, her condition deteriorated further, making daily tasks increasingly difficult. She also reports numbness and tingling in her right arm but denies experiencing pain, recent infections, or injuries.

Nina has a history of hypertension, which she manages with hydrochlorothiazide, and a 25-year history of tobacco use, both of which may contribute to her current condition. An initial consultation led to vitamin supplementation, but this intervention did not improve her symptoms.

With her worsening condition and risk factors, further evaluation is necessary to determine the underlying cause and establish an appropriate management plan.
