A 69-year-old woman with a history of coronary artery disease (CAD), hypertension, hyperlipidemia, aortic stenosis, and an overweight BMI presents with intermittent episodes of diffuse chest pain. The discomfort, which started five days ago, lasts less than five minutes per episode, is triggered by mild exertion, and subsides with rest within 3-5 minutes. She also reports experiencing fatigue and shortness of breath with exertion. She denies any history of smoking, alcohol use, or drug use, and has not had recent infections, fever, weight changes, nausea, or vomiting. On examination, she has mild tachycardia but otherwise stable vitals. A grade II systolic murmur, known from her medical history, is present, but no new abnormalities are noted.
