The patient is a 60-year-old female, a former smoker with a 20-pack-per-year, and PMH of obesity, CAD HTN, and hyperlipidemia who presents with progressively worsening shortness of breath that began about 2 weeks ago with associated 8-lb weight gain and orthopnea. She reports one 1 week of dry, non-productive cough which is worse at night and 3 days of chest pain and fatigue. She denies fever, chills, night sweats, URI symptoms, or syncope. Physical examination reveals tachycardia, +1 bilateral lower of extremity edema, elevated JVP, S3 gallop, displaced PMI, and bilateral lower back crackles upon lung auscultation.
